Important: You will need to complete the following forms before you come for your first appointment.
Forms
These forms can be completed online – allow about 20 minutes in total.
Be sure to provide plenty of information so we can better tailor the treatment to your needs.
Privacy note: We take your privacy seriously. These forms are encrypted, and we store the completed forms on password-protected servers located in Australia. We are collecting this information to uphold good record keeping practices required by law, as well as to gain a good overview of your health situation so that we can better target our treatments to your specific needs. If you don’t provide full information, this may affect the level of care that we are able to provide you.
Other Information
Where relevant, bring photocopies of relevant test results or medical reports (no originals please).
On the day
Wear loose, comfortable clothing – in most cases, clothes that can be rolled up to elbows and knees are fine.
Ensure you’ve had a meal or at least a snack before coming to your appointment.
Cancelling or postponing
You must give minimum of 24 hours notice if you need to change your appointment, otherwise the full session fee is payable. See our Cancellation Policy for more details.
